Please contact the church office if you encounter any problems using this facilities form.

Please use this form to request the use of Trinity's facilities.  When submitted, the form will be emailed to the church office, with a copy going to the email address you provide.  You will also receive a confirmation email from the church staff when your request has been processed.

NOTE: Forms MUST be submitted AT LEAST 3 business days PRIOR to the event.
